New Patient Registration

Both The College Yard Surgery and The Highnam Surgery welcome new patients living within our practice areas. The areas can be viewed  here for College Yard Surgery and here for The Highnam Surgery.

You can register by completing a registration form (provided below). You will need to bring two forms of identification with you, one photographic and one with your current address on. We do not need to see I.D. for children registering with their parents. If you are caring for a patient or are cared for, please bring this to our attention.

If you would like to register with the practice and meet all of the above criteria, please print off the required documents below and complete all sections and return to the practice with original copies of your identification.

Change of contact details

We often need to contact patients and regularly find that the contact details we have are out of date.  Please help us to help you by keeping us informed of any changes, including telephone and mobile numbers.   If you are changing your address we will need to see something with proof of your address on. Please click here to open the form.  Once completed please return to the surgery.
